Sure, a FEW poisons are toxic enough on their own to kill you, somewhat slowly, but if you want the instant death poisons, almost all of them need to be what amounts to distilled down into a purer, far more potent, poison. I noticed that too. When you couple increased monster HP with the effective doubling (at least) of effective PC HP, the total combat day is MUCH longer. Damage output of the PC's goes up too. Fighters get more attacks, spells do more damage, etc... So hopefully it will all balance out. Hopefully ye...

According to the spell description in my book (PH p.70) you can't even change your race! A gnome can only appear to be a different sort of gnome which seems rather odd.

The "Save" part has always bothered me as much as the "Die" part. You drink poison and fail your save, you die, fair enough: that's what you get for drinking poison. However, you drink poison and make your save and you are fine? I'm not so keen on the either one extreme effect versus no effect at all.

Because the player declared something, that doesn't mean the wizard is a brainless robot commited to executing that instruction for the next ten seconds, regardless of circumstances that may arise.
